New & Returning Student Requirements

Students must complete the steps below to register for class.

Uploading health forms and immunization records to the Patient Portal is the fastest and most secure way to provide information to SHS.

New Undergraduate and Graduate Students

Welcome to Gallaudet University! Thank you for choosing this institution as the place to thrive socially and academically while pursuing your collegiate studies. We wish you much success.

There are a few health-related preconditions that must be satisfied prior to attending class.

All new students must:

1. Complete the GU Student Health forms and Tuberculosis Assessment by clicking this link.
2. Student Athletes must also complete both the GU Athletics Physical Exam and Sports Clearance Form and GU Athletics Pre-Participation Evlauation Medical History Form
3. For those 26 years old and younger: submit vaccine records listed on the Health Form per DC Law
4. Either waive or enroll in the university’s student health insurance plan by logging in to Gallagher Student with your Gallaudet email/password and clicking on I WANT TO WAIVE or I WANT TO ENROLL then follow the instructions presented (If you have any issues, please contact the student health insurance email) 

Returning Undergraduate and Graduate Students

Welcome back returning students! 

If you are a returning student registering for fall classes and were an enrolled student during the previous spring semester, you will need to:

1. Upload any updated vaccine records to the Patient Portal
2. Either waive or enroll in the university’s student health insurance plan by logging in to Gallagher Student with your Gallaudet email/password and clicking on I WANT TO WAIVE or I WANT TO ENROLL then follow the instructions presented. (If you have any issues, please contact the student health insurance email) 

If you are a returning student registering for class but was not enrolled the previous 1 – 2 semesters (but less than a 1 year), you will need to:

1. Satisfy any missing vaccine requirements on the GU Student Health Service Form
2. If you are a RETURNING student who has been absent for more than one year, please complete the SHS form in its entirety. 
3. If there has been a change in your health status, then complete the Health History and TB Assessment Forms
4. Either waive or enroll in the university’s student health insurance plan by logging in to Gallagher Student with your Gallaudet email/password and clicking on I WANT TO WAIVE or I WANT TO ENROLL then follow the instructions presented. (If you have any issues, please contact the student health insurance email) 

If you are a returning student registering for class but who has not attended Gallaudet for 1 year or more, you will need to submit the same requirements as a new student.
