To support the health and well-being of students, employees, and our communities, Gallaudet University and Clerc Center requires all Gallaudet and Clerc Center employees, all new and returning Gallaudet students, and all Clerc Center students 12 years of age and older who are eligible for a COVID-19 vaccine to be fully vaccinated. Fully vaccinated is defined here as receiving both doses of a 2-shot series + a booster, OR receiving a Johnson & Johnson vaccine + a booster)
Exemptions may be requested for a bona fide health condition or sincerely held religious belief. Please review and complete the exemption form.
If you have not already done so, please report and upload your vaccination card or medical record to the Student Health Service (SHS) Patient Portal. Be sure the following information is included and legible:
If you are unable to access the SHS Patient Portal, please email a photo or copy of your COVID-19 vaccination card or medical record to SHS Results.
Vaccinations are available on Fridays to all eligible employees and students. Please make an appointment on the SHS Patient Portal.
