Nominations can be made by anyone, but self-nominations are not permitted. A second nominator is required for the nomination to be considered, and the second nominator cannot be a current member of the GUAA. The awards may be given posthumously. All submitted nominations will be considered for up to two award cycles.

Nomination Details

Please describe the nominee’s significant accomplishments and relevant contributions to the awards nominated for. (Please describe why they deserve this award in 250 words or less)
Please attach the following documentation below:
  • A completed online form outlining the nominee’s and nominator’s contact information, along with their relationship to the nominee (nominators cannot nominate a family member, and self-nominations are not permitted).
  • At least two letters of recommendation.
  • The nominee’s resume (if the nominee is an individual; optional, but strongly recommended).
  • A detailed description of the nominee’s accomplishment(s).
  • Supporting artifacts (articles, web links, etc.) that support the nomination.
  • Submissions can be made in writing or in ASL.
