How To Request a Disability Related Accommodation

DSC staff review and process disability related requests for:

• academic & classroom accommodations 

• housing accommodations 

• assistance animals (emotional support animals and service animals)

Meal plan waiver and kitchen access are handled by Campus Services; parking accommodations are processed by the Department of Public Safety (DPS).

All requests for disability accommodations are made through Workday, Gallaudet’s online information-management system. 

Workday Instructions

In the Workday search bar type “Create Request.” Click on Create Request in blue. Choose “All.” Scroll down to Student Accommodations Request and select the type of accommodation you are seeking.

Next: download the specific form(s) depending on the type of accommodation you are requesting.

Have your medical professional who has knowledge of your disabilities complete the form. If you are requesting to bring an assistance animal your animal’s veterinarian will also need to complete a form. 

Once the forms are completed return to Workday/Student Accommodations Request to upload the forms and complete the questionnaire. 

Click submit.

Students may apply for services and request accommodations at any time during their academic career at Gallaudet. We encourage students new to Gallaudet to start this process well before their first term begins. Disability-related information is strictly confidential and handled the same as other medical information.
