IMMUNIZATIONS
DC Law requires any college student 25 years or younger to be up to date with their immunizations/vaccinations.
How you can get a copy of your immunization record:
- Access your patient portal from your home primary care clinician (PCP) and upload a copy to the SHS patient portal at gu.live/portal
- Ask parents or caregivers if they have records of your childhood immunizations.
- Fill out a SHS Release of Information (in the patient portal under forms) to have your PCP’s office fax your immunization record to SHS.
- Check with your previous high school and/or college health services for immunization records.
- Check with previous employers who may have required immunizations.
- Ask your state’s health department for a copy of your immunization if they vaccinated you.
- If you received vaccines from a pharmacy, like CVS or Walgreens, check their portal for a copy of your vaccination record.
