IMMUNIZATIONS

DC Law requires any college student 25 years or younger to be up to date with their immunizations/vaccinations.

 

How you can get a copy of your immunization record:

 

  1. Access your patient portal from your home primary care clinician (PCP) and upload a copy to the SHS patient portal at gu.live/portal 
  2. Ask parents or caregivers if they have records of your childhood immunizations.
  3. Fill out a SHS Release of Information (in the patient portal under forms) to have your PCP’s office fax your immunization record to SHS.
  4. Check with your previous high school and/or college health services for immunization records.
  5. Check with previous employers who may have required immunizations.
  6. Ask your state’s health department for a copy of your immunization if they vaccinated you. 
  7. If you received vaccines from a pharmacy, like CVS or Walgreens, check their portal for a copy of your vaccination record.
